Purpose: Assure maintenance of all minimum standards of Mobile Crisis Management
Make a difference in someone’s life. Work one on one with a person with I/DD helping them to reach goals developed for individual needs to assist them to become as independent as possible. You could be working with them in a private home or a location in the community.

Applicant must be willing to assist with household chores-such as assisting with cleaning and managing of consumer’s apartment. Taking consumer out in to the community to various places for social interactions and errands. Consumer is independent in the bathroom but may need assistance at times with feeding.

Hours/days: 1-1:30 pm- 4-4:30 pm 2- 5 days week depending upon what applicant would like.
Applicant must be willing to transport consumer to YMCA, Library, Parks, go to her church, etc for social interactions. Must assist with maintaining schedule and routine.

Applicant must be willing to take consumer out in to the community for social interactions. Mange pushing a manual wheelchair during long distances. Consumer is independent in the bathroom and with feeding himself.

Implement individual Person Centered Plan involving skills that promote independence while instructing, modeling, assisting the consumer in daily living skills, socialization, leisure and /or recreational activities, educational and vocational skills.

The Hospital Transition Team provides short term services designed to help people maintain psychiatric progress made in psychiatric hospitals or progress related to recovery made in residential substance abuse treatment programs, connect with services and benefits quickly after hospital discharge, and receive support for returning back to their home and community.

MORES is a team-based crisis intervention for children and adolescents ages 3 to 21 years experiencing escalating emotional and/or behavioral needs. MORES provides up to four weeks of follow-up services for individuals who have experienced a behavioral health crisis. The goal of the MORES program is to stabilize the present concern, empower & educate families to monitor, manage & cope with similar situations in the future, establish & build resiliency skills and strengthen the natural support systems of families.
