  • Overview

    Working in an apartment setting with adults with disability or mental illness.  To provide persons supported with quality services and assisting them to become as independent as possible. Transporting them to outing in community, doctors appointment, etc.  Assisting them to reach the goals that have been developed for them.

    Responsibilities

    1. Transport tenants to weekly scheduled appointments such as doctor, grocery, and recreational.  At times this may include out-of-town appointments.  

     

    1. Emergency or crisis response to meet tenants needs if apartment manager is unavailable (i.e. tenant locked out of apartment, etc.) 

     

    1. Collect monthly rent and submit to apartment manager

     

    1. Report changes in person supported condition, family situation, or needs to the appropriate personnel.

     

    1. Make visual inspections of apartment complex and individual apartments on a monthly basis

     

    1. Notify apartment manager of any needed repairs noted by paraprofessional or reported by tenant, and if necessary meet with repair person if tenant is unavailable

     

    1. Get required HUD and MCO documents signed by tenants and return to apartment manager

     

    1. Keep required documentation such as monthly progress notes to support Supervised Living Low services and submit to apartment manager.  

     

    1. Adhere to company policy that prohibits taking a person supported to a private residence without written approval of administration or leaving consumer unattended in a vehicle or at any other public place.

     

    1. Report any allegation of consumer abuse, neglect, or exploitation of consumer to Supervisor to be investigated and an incident report to be completed, along with a report to the local DSS

     

        11.To receive, read, and be familiar with the Employee Handbook regards personnel Policies and Procedures for the agency

     

    1. Perform other duties as required by circumstances.

    Qualifications

    Required knowledge, skills, and abilities:  Knowledge of basic principles of I/DD, ability to work with persons who may be different from me in areas of ability, age, economic status, etc.; empathy and understanding of consumer problems and ability to respond to these problems; ability to determine consumer needs; ability to engage with and build a rapport with the person served.

     

    Requires a high school diploma or GED. 

     

     All employees must have a valid driver’s license and 3 full years of driving experience, no DWI on their driving record for at least the past 7 years, a safe driving record, and an acceptable level of automobile liability insurance.

     

    Must have no substantiated findings on the NC Personnel Registry and not be listed on the HHS/OIG List of Excluded Individual/Entities.  SBI and DMV records must meet the requirements of area mental health contracts.

     

    Experience with persons with mental illness required.
