  • Overview

    To provide habilitation and skill building to enable the participant to acquire and maintain skills, which support more independence to live in community settings.


    Reports to: Service Coordinator/Qualified Professional or Director Supervisor


    This position will currently be for 40 hours per week. Please note that hours are subject to change without notice. 


    Responsibilities Regarding Consumer Rights:

    1. To know, practice, and promote the policies and procedures of Therapeutic Alternatives, Inc., thereby ensuring

            basic rights to all people receiving our services.

    1. To attend meetings and in-services as directed by my supervisor or administrators for the purpose of continuing education

            regarding consumer rights, consumer choices, and consumer outcomes.

    1. To respect and maintain the confidentiality of all people supported and their information to which I am privileged to while

            carrying out my general responsibilities and abide by the HIPAA rules and regulations.

    1. To be polite and courteous to all people supported, guests, and staff of this corporation.


    General Responsibilities of Community Living and Support- Enhanced  Paraprofessional


    1. To receive, read, and be familiar with the Employee Handbook regards personnel Policies and Procedures for the



    1.  Required knowledge, skills, abilities, and specific consumer competencies:  Knowledge of basic principles of I/DD,

           ability to work with persons who may be different from me in areas of ability, age, economic status, socially, etc; 

           empathy and understanding of consumer problems and ability to respond to these problems;

           ability to determine consumer needs; ability to engage with and build a rapport with the person served.


    1. Become familiar with and implement the individual’s habilitation plan such as Individual Support Plan, or to address

           the individual’s identified goals according to the service definition.


    1.  Document the service provided in progress notes.  The basic data included in these notes will include

            observations, consumer participation, and progress, behavior changes, or any pertinent  

            information relating to the consumerAll documentation must be completed on the day the service is

            provided and completed in BLACK INK only.   Use of “white-out” is not acceptable.


    1.  Adhere to company policy that prohibits taking a person supported to a staff’s private residence or leaving consumer

            unattended in a vehicle or any other public place.

    1.  Submit all required documentation by the deadlines specified by the Supervisor.


    1.  Obtain and keep current all training required by the agency, according to the service provided and according to the

            individual’s needs.  Understand that failure to renew training by date due will result in immediate removal from

            schedule until training is brought up-to-date.


    1. Attend all mandatory staff meetings and consumer conferences or give advance notice to Supervisor as to reason(s)

            for inability to attend.


    1.  Receive supervision from Supervisor/Qualified Professional (QP) or Associate Professionals (AP) grandfathered in

            as of waiver implementation of 3/31/12 at least quarterly and as needed either face-to-face, via phone, or group staff



    1. All paraprofessionals and/or associate professionals will be supervised by Qualified Professional accordance with

           G.S. NCAC 27 G .0204(b) (c) (f) and according to licensure or certification requirements of the appropriate



    1. Work only authorized hours unless specifically approved in advance by Supervisor/Administrator.


    1. Report changes in person’s condition, family situation, or needs to the Supervisor.


    1. Report any allegation of consumer abuse, neglect, or exploitation of consumer to Supervisor so it may be

            investigated and an incident report to be completed, along with a report to the local DSS.


    1. Report any destruction or theft of agency property to Supervisor.


    1. Report any possible fraud against the consumer or agency to the Supervisor to be investigated.


    1. Perform other duties as required by circumstances.


    Education:   Must be 20 years old with High School Diploma or high school 

    equivalency (GED) and able to read, write, understand, and follow directions. Must meet the definition for paraprofessional or associate professional as in 10A NCAC 27G .0104(15).  Must have no substantiated findings on the NC Health Care Abuse Registry and not be listed on the HHS/OIG List of Excluded Individual/Entities.  SBI and DMV records must meet the requirements of LME/MCO contracts. General requirements if position requires transporting consumer: All employees must have a valid driver’s license and 3 full years of driving experience, no DWI on their driving record for at least the past 7 years, a safe driving record, and an acceptable level of automobile liability insurance. 


    If position requires transporting consumer in a company vehicle, then all of the above general transporting requirements are applicable as well as an employee must be at least 25 years old vs. 20 years old.  

    If position requires transporting consumer in a personal vehicle, then all of the above general transporting requirements are applicable as well as, if the employee got their license at 16 years old then they must be 20 years old to transport; if they got their first license at 17 years old then the employee must be 21 years old.


    Experience with persons with intellectual/developmental disabilities preferred but not required.  The following trainings are required: CPR, First Aid, NCI A, Client Rights & Confidentiality, OSHA, HIPAA, Community Living and Support Service Definition & Documentation, Person-Centered Thinking, Intellectual/Developmental Disabilities Overview, and enhanced Client Specific Needs Competencies  and trainings are required.  


    CNA: Preferred


    Must have experience with Cellulitis Care and be able to lift a minimum of 75 pounds.
