Make a difference in someone’s life. Work one on one with a person with I/DD. helping them to reach goals developed for individual needs to assist them to become as independent as possible. You could be working with them in a private home or a location in the community.
Assure safety through observing persons supported and documenting status regularly.
Will monitor person’s health and well-being and documentation status on service note. Will report any concerns, and/or safety issues immediately supervisor/qualified professional.
Assist persons supported in completing ADLs, taking prescribed medications, and eating meals.
Required knowledge, skills, and abilities of basic principles of MH/DD/SA.
Will implement the service as indicated in the person-centered plan of the person supported.
Provide daily service note per day that meets documentation requirement according to agency policy.
All documentation must be completed on the day the service is provided and completed in BLACK INK only. Use of “white-out” is not acceptable.
Submit all required documentation by the deadlines specified by the Supervisor/Qualified Professional.
Obtain and keep current all training required by the agency. Understand that failure to renew training by date due will result in immediate removal from the schedule until training is brought up-to-date.
Be familiar with TA, Inc. policies and procedures and the regulations concerning medication administration.
Attend all mandatory staff meetings or give advance notice to Supervisor as to reasons for inability to attend.
Receive ongoing on-site supervision by Qualified Professional.
Report to qualified professional any suspected abuse, neglect, or exploitation of person supported.
Assure person supported rights are met and maintain consumer confidentiality at all times.
To be familiar with the agency’s national accreditation and CABHA requirements
To be familiar with HIPAA regulations and abide by them.
Perform other duties as required by circumstances.
Requires a high school diploma or GED. Auto insurance company requires that an employee be 21 years old, with three years driving experience, and three or less points on drivers license.
Must have no substantiated findings on the NC Personnel Registry and not be listed on the HHS/OIG List of Excluded Individual/Entities. SBI and DMV records must meet the requirements of area mental health contracts.
Experience with persons with developmental disabilities preferred
but not required.
